貌似单纯疱疹病毒性脑炎的MELAS6例报告

摘要：

目的探讨线粒体脑肌病伴乳酸血症和卒中样发作(mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes,MELAS)的临床和影像特征,并与单纯疱疹病毒性脑炎(herpes simplex virus encephalitis,HSE)鉴别。方法回顾性分析6例临床表现酷似HSE的MELAS患者临床影像和病理资料。结果6例MELAS患者均因持续头痛、发热、癫痫样发作和智力下降被误诊为HSE;但还表现有视听力障碍、身材矮小、运动不耐受和(或)内分泌异常(糖尿病、甲状腺功能低下等),影像以大脑后半部颞枕顶叶皮层病灶为主,单侧或双侧受累,可与HSE鉴别,肌活检可见破碎红纤维。结论临床表现酷似HSE的病例,应与MELAS鉴别,确诊需肌活检。

Objective To investigate the characteristics of clinical manifestations and imaging of MELAS and to differentiate it from herpes simplex virus encephalitis (HSE). Methods The clinical,neuroradiologic and pathologic data of 6 cases of MELAS that masquerading as HSE were studied retrospectively.
